Faith Formation Registration

• Contact John Masterson if you prefer a hard copy registration form.
• Children must be 4 years old by September 1, 2024 to attend PK Faith Formation.
• Children must be registered for Faith Formation in order to prepare for Sacraments of Reconciliation, First Eucharist, and Confirmation.
• Class Options:
  • Option 1: Sunday 9:15AM-10:15AM (Catechesis of the Good Shepherd 3-5 year olds)
  • Option 2: Sunday 9:15AM-10:15AM (PK-Grade 5)
  • Option 3: Wednesday 5:30PM-6:30PM (PK-Grade 5)
  • Option 4: Wednesday 6:30PM-8:00PM (EDGE  Grade 6-8)
  • Option 5: Sunday 6:00PM-7:30PM (Confirmation)
  • Option 6 Monday 6:30PM-8:00PM (Post Confirmation 1st & 3rd Mondays)
